Corah's dental anxiety survey
udyererueueeueuueuuueudhgCorah’s Dental Anxiety Survey (CDAS)
This is a four-item measure developed in 1969, where respondents are asked about
four dentally related situations and are asked to indicate which option is
closest to their likely response to that situation. The CDAS is widely used,
but has been criticised for exhibiting a range of scores too narrow to be used
effectively in clinical studies

I would look forward to it as a reasonably enjoyable experience
I wouldnt care one way or the other
I would be a little uneasy about it
